Online form submissions are handled by a secured mailbox and actioned by our staff, then archived or securely disposed of in line with our retention obligations.
We share your information with your referring doctor, and, where relevant, with Medicare/Services Australia and other health professionals involved in your care. We do not disclose your information overseas as a matter of course. We may disclose information where required or authorised by law.
